What to Bring
Copies of the following documents are needed prior to admission:
- Medicare Card - Part A & B
- Medicare Card - Part D (Prescription Card)
- Insurance Card
- Medicaid Card
- Social Security Card
- Durable Power of Attorney / Guardianship Document
- Advance Directives / Living Will
We also recommend that you bring 5-7 days’ supply of comfortable, seasonally appropriate clothing and shoes. Please bring personal belongings such as pictures, photo albums, writing supplies, pillows, throws…anything that will help you feel at home. All personal items need to be marked with your name You may also have a phone installed in your room for private use, by contacting AT&T.
